The game forces the player to choose: stop playing (resulting in a "bad end" where the protagonist commits suicide) or continue killing digital pigs (resulting in a "worse end" where the protagonist loses all humanity, becoming a feral, obese creature living in a literal pig stye, eating raw garbage).

The gameplay of "Buta no Gotoki" diverges from the traditional Yakuza series by incorporating turn-based combat, a departure from the live-action combat system of its predecessors.
